If you prefer to report your grievance via phone, please call 844-918-3400 and either remain anonymous or request a return call.
If you would like to submit a report online, please visit www.hotline-services.com. There will be a field listed as “Organization Name”, and you will simply type “Surpass Behavioral Health”. Click “Submit New Report” and follow the prompts to complete.
